The hypertension Challenge

Usually, high blood pressure has no symptoms at all. That is why it is often called the "silent killer." Millions of people have high blood pressure and many do not even know they have this serious condition.

Why such situation?

Patient responses to diagnostic cardiovascular tests, to interventional devices like pacemakers and to pharmacological treatment with drugs are highly variable and unpredictable, and are often composed of changes in blood pressure, hemodynamic, arterial stiffness ,ANS level activity and SpO2 % . Today, their assessment is based on ECG measurements and intermittent standard upper arm NIBP cuff data that misses most if not all of the above responses.

The 2003 European Society of Hypertension guidelines on the management of hypertension acknowledge the potential role of arterial stiffness measurement in clinical management and have prompted the publication of a consensus document on the measurement of central blood pressure and hemodynamics. The guidelines concluded: “Broader implementation of these techniques into routine care seems inevitable”.

Blood pressure is continually changing depending on activity, temperature, diet, emotional state, posture, physical state, and medication use.

Repeated measurements are important. A single high measurement does not necessarily mean high blood pressure. On the other hand, a single normal measurement does not necessarily mean that non high blood pressure. Many people become nervous at the doctor's office and have higher readings that they normally would at home. This is called white coat hypertension.

NOTES ! It is important to note that the recommended diagnostic evaluation is based on a consensus and should not be considered evidence-based. A complete history, physical examination, and limited diagnostic testing (urinalysis, complete blood count, potassium, sodium, fasting glucose, creatinine, total cholesterol, high-density cholesterol, and electrocardiogram) are recommended once the presence of hypertension has been confirmed.
